Narrative
At 23:41 on 10/14/25, pcc received an alarm on a gas detect at the arbuckle ii dayton pump station. An area operator was dispatched and upon arrival identified a leak originating from underground. The station was shut down, and the suspected segment was isolated and de-inventoried. Upon excavation, non-destructive evaluation testing determined the source of the leak was a crack in a fillet weld on flare piping. The complete weld was removed, and a new weld was made per oneok welding procedure. The crack was evaluated and is attributed to incomplete penetration in the first pass combined with an element of mechanical stress. Nrc was notified on 10/17/25 once determination was made that the cost would exceed $50,000.
