Narrative
Station operator arrived at 14:30 pm on 10/15/2025 for a station check and discovered that release from a needle valve was in progress. This is a 1/2� needle valve on the alternate test port fitting on the tank manifold inlet surge relief valve. Failure resulted in a 36-gal release of crude. As a permanent containment and repair, 1/2� fitting and ball valve was installed to replace the needle valve. Some of the needle valve was missing. Being a small part, it was not found and probably sucked up when the hydrovac operations were being conducted to remove the contaminated soil. Therefore, it is unknow how the needle valve failed. This was not a piece of equipment that is remotely monitored and the small volume of discharge would not have been detected from the operations center.
