Narrative
Oneok technician arrived on site for semi-annual valve inspection and discovered a stem packing leak. Technician notified local supervisor of the situation. Osro was dispatched to assist with clean-up efforts. Operations injected additional valve packing to stop the packing leak. The entire valve was removed and replaced with a new valve setting with a stainless-steel stem. The nickel-plated carbon steel stems are a known problem due to holidays in the nickel plating on the carbon steel stems allowing corrosion to occur resulting in stem packing damage. There are currently ongoing mitigation efforts to replace all similar valves.
