Narrative
On 8/20/2025 at approximately 07:09, a landowner reported they struck the 21001.08 - 10inch zephyr to talpa line with a ripper attachment on their bulldozer while cleaning out a creek for a culvert install. The line was shutdown and valves isolated. The line was exposed and a dent with through-wall puncture was found. Approximately 200 barrels of crude oil was released. Osro and contractors cleaned-up the released product and removed impacted soil. The leak was repaired with a plidco clamp. On 8/21/2025 at approximately 02:00 the line was restarted. The cause of the accident was due to excavation damage by a third party. To prevent reoccurrence, a failure to comply letter was sent to the landowner.

Narrative
On 8/20/2025 at approximately 07:09, a landowner reported they struck the 21001.08 - 10inch zephyr to talpa line with a ripper attachment on their bulldozer while cleaning out a creek for a culvert install. The line was shutdown and valves isolated. The line was exposed and a dent with through-wall puncture was found. Approximately 200 barrels of crude oil was released. Osro and contractors cleaned-up the released product and removed impacted soil. The leak was repaired with a plidco clamp. On 8/21/2025 at approximately 02:00 the line was restarted. The cause of the accident was due to excavation damage by a third party. To prevent reoccurrence, a failure to comply letter was sent to the landowner.
