Narrative
Fhr was notified by a 3rd party operator of a potential release at the 501e mustang skid. Fhr operations personnel arrived onsite at approximately 12:01am on 8/14/25 and observed a fugitive emission with a small ice ball. The tubing was isolated and was replaced by ~03:00. The initial release estimate was determined by calculating how much product was contained in the 4 prover arm, valve body, and thermal body relief tubing (approximately 1 gallon of propylene). The root cause investigation began in the field later that afternoon. It was discovered that the tubing connection could have seen pipeline pressure since the release point on the tubing was located after a check valve on the tubing assembly. At ~6:40 pm, the release volume was re-calculated taking the new information into account, and it was determined that the release volume could have been approx. 52 gallons. Fhr gathered the remaining required information and reported the release to the trrc at ~ 7:40 pm. The tubing has been sent to a 3rd party lab as part of the root cause investigation.
