Narrative
Approximately 55 barrels of crude oil were released from a tank booster at joe mabee station. The date and time of the release (part a q4) are indicative of the earliest possible time the release could have started based on the last date/time the incident location had been visited and confirmed to be no release of crude oil. The cause of the release is attributed to equipment failure as a result of the pump manufacturer installing a rubber gasket instead of a stainless-steel gasket. To prevent future incidents, the gasket was replaced, and conversations about preventative measures are being discussed with the third-party vendor. Supplemental: corrected the date/time of notification to the texas railroad commission.
