Narrative
On july 26, 2025, at approximately 5:13 pm cdt, technicians at the taft terminal arrived for a pigging operation and discovered the sump overflowing. The control center was promptly notified, and the line was shut down and the facility was isolated. Management was notified and cleanup activities were started. Approximately 600 barrels of crude oil was released with 53 barrels being returned to the system. The remaining 547 barrels was part of contaminated soil and was properly disposed of at a disposal facility. The nrc was notified on july 26, 2025, at 6:04 pm cdt (report # 1438054) and the 48-hour update was made on july 28, 2025, at 12:21 pm cdt (report # 1438187). The railroad commission of texas was notified on july 26, 2025, at 6:27 pm cdt (report # inci 3376). The internal investigation determined the immediate cause as a failure to adhere to pipeline pigging procedures. Specifically, the pigging launcher 2-inch drain valve was left in the open position. The open drain valve permitted product to flow into the sump which caused the sump to overfill.
