On june 2, 2025 at approximately 12:00 p.M. Cdt, a landowner notified austen pitts that there was a leak on the 4.5" texok CO2 pipeline row downstream of mlv8 at mp37.25. Mr. Pitts proceeded to the location to confirm the line failure at approximately 1:00 p.M. Cdt. The failed section of pipeline was isolated and the beaver county, oklahoma sherrif's office was notified of the plan to blowdown the failed section at 1:20 p.M. Cdt on 6/2/25. The planned blowdown was completed at 4:40 p.M. Cdt. Removed 13' of 4.5" pipe on 6/4/25 and welded in-place replacement section of pipe on 6/5/25. The line resumed operations on 6/12/25 at 5:00 p.M. Cdt. There were no injuries reported and no evacuations or public notification was necessary. Metallurgical engineering services, inc. (mes) performed a failure analysis which stated: "based on the comprehensive failure analysis conducted on the subject four-inch nominal pipe, the primary root cause of the failure was identified as weld defects within the circumferential girth weld, which were preferentially corroded from the inner diameter through the weld heat affected zone (haz) until the joint was compromised." based on their failure analysis, mes identified the primary root cause of the failure as, "...Weld defects within the girth weld haz, specifically undercuts, incomplete penetration due to poor alignment, and slag inclusions. These defects led to preferential corrosion attack, exacerbated by the presence of chalcogens, which resulted in the through hole failure. All relevant test outcomes supported these findings, confirming that the failure originated from weld-related issues rather than the base metal composition or mechanical properties."
