Narrative
During hydro excavation work near the ln02 prover at krotz springs, a vacuum hose momentarily plugged with soil, which quickly broke free and cleared. This resulted in a sudden change in pressure causing the hose to jump and move. This move resulted in the hose landing on top of a 3/8' pipe fitting causing it to bend from a 90� angle to a 45� angle which resulted in a release. Operations personnel were notified and the acc. The acc shutdown krotz springs ln02 injection and isolated the prover. A total volume of 11.5 gallons of diesel was released. The fitting was repaired and all product was removed the contaminated area. All necessary internal and external notifications were made. An incident analysis (ia) will be conducted on this event.
