Narrative
05/23/25 at 0905 enterprise houston control center was notified by enterprise mont belvieu shift supervisor of an iso-butane leak at the morgan's point facility. (29.681638, -95.013962) all movements at morgan's point were shut down, lines were isolated, and release was contained within facility property. There were no injuries, fatalities, road closures involved with the incident. Due to estimated release of product greater than 5 gallons a telephonic notification to the railroad commision of texas pipeline safety department was required. Internal investigation concluded the apparent and root cause of the release was due to 3/4" threaded pipe nipple with a mismatched schedule rating. Component was replaced with correct schedule rating and line was put back to service. Update 06-26-2025 subpart g6 sub-cause unchecked improper installation and checked excessive vibration to align with 3rd party analysis which states "the fracture of the nipple was the result of fatigue that initiated at the thread root of the last engaged thread."
