On may 21, 2025, eagle sky patrol notified austen pitts that there was an anomaly on our pipeline row. He proceeded to the location indicated by the sky patrol to confirm the line failure. The area supervisor, josh burkhart, was contacted. An operational plan was discussed to shut-in mlv6 and mlv7 to contain and blowdown the pipeline. Shortly after, mlv5 and mlv8 would be shut-in to complete isolation of the area. The state biologist was notified that austen would be entering the beaver river wildlife management area (wma) to investigate the pipeline failure. Once the pipeline failure was confirmed, austen contacted the beaver county, oklahoma sheriff�s office to inform them that capturepoint was investigating a line failure in the wma, and there was no immediate danger to people or property from the observed plume and no evacuations or public notification was necessary. It was determined that the line failure appeared to be along the seam of an erw weld of the pipe that was manufactured and installed in 1968. The welder replaced 27'8" of the 4.5" pipe and plans were discussed and executed to place the line back in service. Notifications were made to the beaver county sheriff�s office and control room management contractor that purging and valve openings would be taking place at 2125 on 5/23/25. The new line in the wma was visually inspected the morning of 5/24/25 and the capture plant compressors started back up at 0830 on 5/24/25. The tx-ok line that failed in the wma was handled safely and in a timely fashion. The proper actions were taken to locate and inform others of the failure. The necessary people and equipment were brought in to investigate the failure and determine the replacement of the line would be needed. Metallurgical engineering services, inc. Performed a failure analysis and determined that the failure was "primarily attributed to a combination of preferential corrosion at the outer diameter and inherent defects in the lf-erw seam." the subject failed pipe segment consisted of a 20.5' length of pipe with a 4.5� outer diameter (4� nominal diameter), with a wall thickness of 0.136". Following removal of the back tape at the center and each end, inspection of the pipeline segment revealed primary failure as a through-hole indication, located within the longitudinal weld seam, at a location approximately 145 inches from the downstream cut end. The through hole was located within a bent region of the pipe, 5.5-inches from a circumferential girth weld. Based on field label, failure was located at the 9 o'clock position, as viewed from the upstream direction. The exterior of the pipe had been stripped of the coating system, with residue black regions, consistent with a tar or mastic based coating. The failure location exterior was covered in light layer of orange iron oxide. The failed pipe section was bent ~10-degrees, presumably due to forces from expelled CO2 buildup. The through hole was located within the longitudinal weld seam, approximately 5-inches from a circumferential girth weld. From the exterior surface, the through hole was oval shaped and 0.2"x0.1" in size, with cracks at each end that extended 0.5". The exterior surface surrounding the crack showed a rough 'woody fracture' morphology with longitudinal lines of material loss with intermittent cracking. A woody fracture appearance may indicate an erw seam region that is affected by clusters of inclusions and/or small discontinuous hook cracks.
