On 5/3 2025, wes personnel was responding to a work order when he came across a leaking mov due to what appeared to be loose packing. Temporary containment was immediately put in place and the screws on the packing were inspected, tightened and the leak immediately stopped. The valve and packing were put under monitoring and inspected for 14 calendar days to ensure the packing remained tight and prevented any further leakage. Approximately .29 (12 gallons) of crude oil were released to ground from the stem of a control valve on the block 3 oil header ruck trap junction to prevent recurrence, after a 30 day follow up inspection, the valve contractor was contacted to re-pack the valve, which occurred on 6/5. This incident occurred on a western midstream subsidiary pipeline, dbm oil services llc
