On october 22, 2025, at 6:30 a.M. While investigating line shut-ins and compressor shutdowns due to high o2 issues an operations (ops) tech observed that the headlee discharge line had 40 psig when the normal operating pressure is 1250 pisig. The employee notified the ops supervisor who then verified the line pressure with another ops tech located at the lupin station. At 6:40 a.M. The ops team began a leak investigation on the 7406642 - lupin to headlee. At 09:37 a.M. Field personnel pinpoint the location the leak. The line was shut in and field personnel began to investigate the severity of the leak. At 11:15 a.M. Operations made an initial determination that the leak may exceed the reportable threshold and notified DOT compliance. The nrc notification was made at 12:06 p.M. Txrrc notification was made at 12:15 p.M. The investigation is ongoing.
