Narrative
On 4/25/25 during a typical routine pipeline row patrol of the sfpp ls-111 tipton valve, km employees discovered the barbed wire above the entry gate was breached and the body bleed port on the block valve had been opened. As a result, approximately 12,348 gallons of turbine product were released onto the ground. Operations was notified, the location was secured, and all relevant agencies were notified. There was no standing product, so multiple methods were utilized to calculate the release amount including: soil boring and lab analysis, gain and loss calculation, orifice size of opening and pressure readings calculated. Investigation continues along with remediation analysis. Nrc notifications: 1429443 initial nrc notification 1429455 PHMSA criteria 1429523 48hr update
