Narrative
Columbia gas transmission llc (columbia), a tc energy entity, operates and maintains forrest hills regulator station located in brooksville, kentucky. On tuesday october 14th, 2025, 3rd party nde technician (contractor), and a columbia m&r technician were hand prepping corroded surface areas of a 1-inch diameter fitting for evaluation. During hand preparation the fitting began leaking. Columbia's area operations manager was contacted and instructed the columbia m&r technician to evacuate the building and leave the doors open for venting. A columbia pipeline technician arrived on site to perform lel readings around the building. After assessing the event, confirming that no gas was being detected beyond the meter building, it was determined that potential impacts to residential life and property did not warrant residential evacuation at the time. The facility was manned around the clock with hourly instrumented leak sweeps from october14th to 19th. The leak was stable with corrective action including implementation of a pressure reduction to 320 psig. The leak source was identified at the side wall of the 1-inch diameter fitting. A root cause investigation is underway. The fitting has been repaired, and the facility was returned to service on october 19th, 2025.
