On 04/30/2025 at approximately 1300 hrs, tank mixer seal on crude oil tank 221 failed and released approximately 5 gal (0.12 bbl) of crude oil to immediate area under the mixer onto rock and soil. The leak was discovered between operator rounds and immediately stopped, contained and cleaned. No third parties were involved as part of repair or cleanup. Investigation: a significant contributing factor was a failure of the mixer bearing (loose set screws) which caused a misalignment between seal and shaft, causing the minor release of crude oil.
