On 4/8/2025, at approximately 08:40, an operator employee discovered a leak on the 16inch valve on tank line 1979 at midland tank farm. The leak was immediately isolated. Approximately 12 barrels of crude oil was released. Cleanup contractors removed product and remediated impacted soil. The leak was repaired by replacing the valve. The line was put back in service on 04/11/2025. The cause of the accident is still under investigation. A supplemental report will be submitted once the investigation is complete. Supplemental & final: the investigation determined the cause of the accident was due to internal corrosion. The leak coincided within a region of internal corrosion located along the bottom of the valve body. A pit in the flange material adjacent to the leak location was also observed, which aligned with pre-existing manufacturing voids. Additionally, a second pit was identified within the adjacent flange material of the valve, further contributing to the localized corrosion and eventual leak. To prevent reoccurrence, nde examine one other valve by same manufacturer with same approximate manufactured date to ensure no thinning of the body was occurring.
