Narrative
At 16:31 on 9/12/2025 psv-5201, a discharge over-pressure relief valve on unit 5200 at seneca compressor station began to vent gas below the desired activation setpoint. This activation was due to a pipe nipple failure on the field test port. The scada system alerted occ and the mechanic on call for the station when it detected hi gas levels in the compressor building. The mechanic on call received the alarms and began driving to seneca station. While the mechanic was on the way the operations team asked occ to issue a remote stop command to the two units running at the station, unit 5100 and 5200. This command stopped the unit and commanded the suction and discharge valves closed, stopping the flow of gas to the psv. When the mechanic arrived on site, he discovered the broken pipe nipple and removed the assembly and replaced it with a plug. The unit was then checked and made available for service.
