On 9/12/2025 at approximately 19:32, up-24 ruptured at station 125+05 near the drip and launcher / receiver at the uw-204 well pad in forbes state forest which is located west of farmington, pa in fayette county. Gas control received initial indication of the rupture from scada pressure drop alarms on up-2, up-5, and up-7 at 19:32. Up-24 is an 8 inch diameter transmission pipeline in storage function approximately 2.3 miles long and transports gas between other transmission pipelines in storage function and uw-204 which is an underground storage well in north summit storage. The maximum allowable operating pressure of up-24 is 3377 psig. Up-24 was in normal operation with an operating pressure of 3176 psig and no flow when the incident occurred. At approximately 20:20, operations personnel called gas control and confirmed that natural gas was blowing in north summit storage. Fayette county 911 called gas control at approximately 20:31 and provided a similar notification. Operations personnel manually closed valve up-24 upstream of up-24 at approximately 20:55 and manually closed the master gate valve and lead lines on uw-204 downstream of up-24 at approximately 21:20 to complete the isolation of up-24. Gas control also initiated commands to remotely close valves up-5 and up-116 to support the isolation of up-24 at approximately 20:57. Gas ceased blowing from up-24 at approximately 21:29. The rupture resulted in the longitudinal and then circumferential failure of pipe going to the drip; an approximately 25 ft by 30 ft hole in the ground approximately 12 ft to 15 ft deep; approximately 14 ft of pipe folding up and coming out of the ground; approximately 1.5 ft of pipe being ejected approximately 73 ft away from the hole; damage to the drip and associated piping; damage to the launcher / receiver and associated piping; damage to a nearby aboveground brine storage tank at the uw-204 well pad and associated piping; approximately 26,067 mcf gas loss from up-24; approximately 3,092 mcf of gas loss from uw-204; and approximately 0.05 barrels of brine spilled to the ground. The gas did not ignite and there were no fires, injuries, fatalities, or evacuations. Initial notification (#1442611) was made to the nrc on 9/12/2025 at 20:45. Follow up notification (#1442677) was made to the nrc on 9/13/2025 at 20:50. The isolation of up-24 remains in effect until the following remediation actions are completed: the section of up-24 containing the rupture is replaced; the drip and associated piping are replaced; the launcher / receiver and associated piping are replaced; and the nearby aboveground brine storage tank and associated piping are replaced. Cleanup of the soil where the brine spilled was completed. Four pieces of up-24 pipe were cut-out at the rupture and were sent to kiefner & associates for failure analysis. The failure analysis report from kiefner is pending, but the preliminary indication is that internal corrosion was the apparent cause. As a precautionary measure, the integrity of the uw-204 wellhead and annular vent were inspected and it was confirmed that uw-204 can be safely returned to service once the remediation actions for up-24 are completed and up-24 is returned to service. A supplemental incident report will be submitted once up-24 is returned to service.
