On 02/10/2025 at approximately 13:40, crude oil was discovered leaking from tank mixer "d" on tank 1570. The leak was immediately reported, mixer isolated, and leaked stopped. The leak was coming from a failed tank mixer seal. Approximately 0.3 barrels (12 gallons) of crude oil was released within secondary containment. Contractors cleaned up remaining product and removed contaminated soil. The failed seal was replaced and tank operations resumed. Supplemental & final: the mixer assembly was examined and no abnormalities noted. The cause of the accident was due to a non-threaded connection failure. The tank mixer seal failed from routine wear.
