Supplemental final update: updated apparent cause to reflect actual root cause of incident determined in investigation. � incident summary: on saturday march 29th pump p-206 had a seal fail around 9am. The pump was isolated, locked out, and suction/discharge piping drained. The system ran for ~12 more hours on saturday utilizing two different mainline pumps with no issues. On sunday march 30th a batch on the same system started at 8am using the other two mainline pumps. A station operator stayed around the pumps until 8:15am and everything was running as normal. The station operator returned at 10:45am and found the seal pot overflowing. Investigation root cause: after the seal fail alarm on saturday morning the pump stayed locked out with the seal fail alarm latched. Unbeknownst to operations the suction valve of the pump was leaking by. This allowed the piping that was initially drained to refill with product. When a batch started at 8am sunday the pipe section was full of product began to pressurize and leak by the failed seal, overflowing the seal pot. The seal fail alarm did not go off because it was still latched from the alarm saturday. Through investigation it was discovered that the seal pot drains were undersized. Preventing re-occurrence: seal pot drain lines will be upsized so that more product cannot enter the seal pot overflow than can be drained out. Programming changes will also be made to the seal fail alarm to stay active even in a latched state to detect a failed seal that leaks by after the initial alarm.
