Narrative
On june 19, 2025, epng determined that a valve associated with the emergency shutdown (esd) system at the washington compressor station was found in a partially closed position following an esd event that occurred on june 15, 2024. Due to flash flooding in the vicinity of the facility, epng personnel were unable to safely access the station to perform an onsite assessment until june 19, 2025. As a result, the cause of the esd event and associated valve position could not be investigated until that time. Epng has initiated an internal investigation, including a formal root cause analysis (rca), to identify the failure mechanism and contributing factors related to the valve malfunction. A supplemental final report will be submitted upon completion of the investigation.
