Narrative
Operations management received initial notification of this incident via a fire department scanner report on 6/8/25 at approximately 18:00. At this time, it was unknown if the incident involved an egts asset as there are other pipeline and underground well operators in the area so operations management dispatched operations personnel to make this determination. After picking up a utility terrain vehicle at southern area headquarters, operations personnel arrived on-site in the nearby vicinity of the incident on 6/8/25 at approximately 18:38. At this time, it remained unknown if the incident involved an egts asset so operations personnel began the process of manually isolating bw-9832 by closing valves (a gate & e gate) as a precautionary measure. Bw-9832 is an underground natural gas storage well in kennedy storage and is located approximately 2 miles northwest of valley chapel, west virginia. The closest road to bw-9832 is mare run road and bw-9832 is connected to the h-19366 pipeline. Operations personnel completed the isolation of bw-9832 on 6/8/25 at approximately 18:55. Following the isolation of bw-9832, operations personnel confirmed discovery that the incident involved bw-9832 on 6/8/25 at approximately 19:04. A tree had fallen and severed the 4� lead line and 4� well line associated with bw-9832. It is believed that this occurred on 6/8/25 at approximately 17:46. The operating pressure of bw-9832 at the time of initiation of the incident was approximately 924 psig and bw-9832 was in the injection phase. The maximum operating storage pressure of kennedy storage is 1050 psig. The natural gas that was released as a result of the incident did not ignite. One residential building with two occupants was evacuated as a precautionary measure. No injuries were sustained and there were no environmental impacts as a result of the incident. Initial telephonic notification was made to the national response center on 6/8/25 at 20:03. The national response center assigned incident report #1433512 to the initial notification of the incident. A follow-up telephonic notification was made to the national response center on 6/9/25 at 21:48 to confirm the initial telephonic notification. The national response center assigned incident report #1433620 to the follow up telephonic notification. A temporary bridge plug was installed on bw-9832 to facilitate the testing of the integrity of the wellhead connections on bw-9832. The testing of the integrity of the wellhead connections on bw-9832 was completed on 6/23/25 and it was determined that bw-9832 can safely return to service once the repairs to the 4� lead line and 4� well line are completed. A supplemental incident report will be submitted once bw-9832 is returned to service following completion of the repairs to the 4� lead line and 4� well line.
