Narrative
A third-party party excavator (meer electric) struck and damaged an atlanta gas light company (aglc) 2-inch plastic gas main while horizontal directionally drilling to install a new water service to 22 habersham way, atlanta, ga. The escaping gas traveled along the bore path from the main to the outside building wall of the residence where the gas ignited, resulting in extensive fire damage to the residence. Upon arrival, aglc personnel worked with emergency responders to secure the area and monitor the surrounding area for gas. Gas was shut off to the damaged area by performing two squeeze-off operations on the two-way feed main. The excavator was performing the excavation activities under an emergency locate ticket and did not pothole and spot the natural gas facilities prior to commencing excavation activities. The gas main was correctly marked at the point of damage. On the day of the incident, aglc recognized the potential damage estimates likely met or exceeded the $149,700 threshold and thus met the criteria for being reportable to the nrc. However, the company did not have reason to estimate the damages to meet or exceed the $1 million threshold for csb reporting. As a result, aglc reported this incident to the nrc under report #1443076 and the 48-hr follow up filed under report #1443201 but did not report it to the csb. On october 14, 2024, after learning that the homeowner�s insurance company reserved the property loss at $3 million, agl notified the csb of the incident.
