(see below for update on supplemental report) national grid had previously performed an inline inspection of a portion of the 30" 350 psi jamaica bay line transmission pipeline in brooklyn, ny, and was preparing to return the pipeline to normal operating status. One of the steps was to insert a completion plug into a 2" thread-o-ring (tor) operating at 310 psig. During this process, a failure from an unknown source occurred, resulting in an unplanned, accidental release of natural gas. A company employee was injured and hospitalized as an inpatient; and a second employee was treated at a hospital. The fitting and materials believed to be involved in this incident will be cut out from the 30" pipeline and tested by a third party lab to help determine the apparent cause. *** supplemental report issued 7/24/2025: the 30" pipeline was shutdown during the weekend of may 31 and the section in question was removed for further examination and analysis. (cause still unknown). A replacement section was installed and the 30" pipeline was gassed back into service on 6/4/2025. No additional costs are associated with the emergency response. The section was cleaned and then sent to an independent metallurgical laboratory on 6/4/2025. Analysis is ongoing. . Entered date/time of restart (section a question 16a and b).
