Narrative
On june 23, 2025, at approximately 16:32 edt, enbridge gas ohio received notification of a fire/explosion event at a structure at address 7051 garden valley avenue in cleveland, ohio. The structure is a 3 story 44-unit apartment complex with approximately 150 residents. Enbridge personnel arrived on scene at approximately 17:05 edt. The responding enbridge employee located an outside meter on the building property and shut it off before calling for further personnel on scene. It was determined that the meter shut off was supplying gas to a neighboring building. An inside meter on the north side of the subject building was in a maintenance room behind a locked door. With fd assistance enbridge gained access to the meter and was able to shut it off at approximately 20:35 edt. Enbridge performed leak detection activities around the building to the extent possible as it was still an active fire with numerous first responders on scene. At approximately 23:35 edt, the fire was under control, and all residents were accounted for, so the fd and enbridge cleared the scene to resume investigations in the morning. It was communicated to enbridge that 5 members of the public had been transported to the hospital due to injuries sustained in the fire (on july 11th, 2025, it was communicated to enbridge that one of these victims died in the hospital due to injuries sustained in the event). After reconvening on the scene the following morning it was determined that there was a second inside meter at the south side of the subject building near the location where the explosion had occurred that could not be accessed due to the partial collapse of the structure above it. Enbridge crews located the curb box and shut off gas to this meter to make safe for debris removal activities. Enbridge then cut both service lines to the inside meters and performed a pressure test at operating pressure. The service line on the north side of the building showed no pressure drop over the course of the test. The service line at the south side of the building showed a drop of 3oz of pressure over the course of a 10-minute test. Enbridge then removed and took into evidence the inside portion of this service line, including the meter valve, which showed signs of damage from the explosion, fire, and building collapse. A second test was performed of the service line to the top of the service riser and no drop in pressure was observed. After the building debris was removed, enbridge was able to locate and recover its meter at this location. Enbridge will perform further testing on the recovered meter and inside portion of the service line recovered and removed from the site to determine if a release of jurisdictional gas occurred prior to the incident that could have contributed to the fire/explosion. At this time enbridge has made no determination that a release of gas from a jurisdictional facility contributed to or caused the incident. This investigation remains ongoing and supplemental notifications will be provided as appropriate.
