Narrative
On 12/02/24 @ approximately 10:45 am, product was discovered dripping from a leak detection jar near unit 2. Unit 2 was then shut down. The leak detection jar was removed, emptied, cleaned and replaced.product continued to drip into the leak detection jar. upon further investigation, product was found in a 24" well near unit 2. Notification was then made to shut-down and isolate the booster station. Spill response personnel were mobilized and when they arrived, began exploration excavation around unit 2. Once fully excavated, the cause was determined to be a stem seal leak on unit 2 check valve. repairs were completed and the booster station was returned to normal operations on 12/03/24 all released product was contained on cpc property. Update 2/3/25 an investigation was completed, and the cause has been updated. The investigation revealed the point of product release was a damaged o-ring on the unit 2 check valve leak detection jar. Contributing factors identified were an upstream unit 2 check valve stem seal leak that allowed product to flow to the leak detection jar and a downstream drain line check valve failure that prevented pressure and product to flow to the sump. With pressure and product building from both directions, an already damaged o-ring on the leak detection jar allowed product to be released.
