Narrative
On 12/03/24 at approximately 11:28, a diesel fuel leak was discovered at montello pump station during maintenance activities. While readying line 6 and line 9 for hydro-test, contractors opened the first flange, introducing air into the pipe, which resulted in product flowing out of the open 10-inch pipe in the 600 manifold area. Product flow was reduced so a 10-inch blind flange could be installed. The blind stopped the flow and isolated the line. Approximately 30 bbls of diesel fuel was released. An onsite vacuum truck responded to collect free product. An osro was mobilized for additional clean-up. The investigation determined that the leak occurred due to the line not being vented at the high point. To prevent reoccurrence in similar situations, an inspector or company representative will verify the drained volume vs line fill comparison prior to completion of the drain down.
