During a facility inspection an operator found a leak around a 4-way prover valve at colorado city terminal. The operator immediately called their supervisor to report the leak. It was determined the bottom flange nuts had backed off causing the release. Discussions with the valve company confirmed that the valve was properly installed per the manufacturer's specifications in 2023. Local ops locked out and tagged out the prover and performed a thorough inspection of the 4-way valve. Soft goods and internal mechanical components did not show any signs of wear, indicative of good working condition. The valve was reassembled and reinstalled. Per recommendations from the valve company, thread lock was applied to the bottom blind flange studs to ensure the nuts would not back off. Local ops conducted several manual proves and were onsite to monitor the system throughout the entire duration of the prove to inspect for leaks. There were no leaks observed, and the prover was approved for return to service.
