Narrative
Cause of the leak is the result of a mechanical failure on the discharge side of the pump transferring crude oil from break out tanks to the lewiston 16". Lambda calculates, that approximately 350-barrel loss had occurred (350 barrels in containment and 5 barrels outside of containment). The timeline of events occurred as follows: ' october 30, 2024 ' truck driver ken taylor notified greg davis with lambda energy gathering, llc of leaking oil at the junction station at approximately 1154 (11:54 am est). He started lambda energy gathering, llc's emergency response procedures, and notified the pipeline manager, other field technicians. ' greg davis and matt allen were working together at lewiston station. Matt responded to junction station to confirm leak. Arrived @ junction station at 1210 (12:10 pm est) ' matt allen immediately contacted gfl environmental (formerly northern a1 services), beckman production to supply vacuum truck to remove free product, and keo services to provide equipment. ' gfl environmental services (formerly northern a-1 services) was working at lewiston station for lambda energy with a hydrovac (vacuum excavation truck). Lambda personnel pulled them and sent them to the spill to begin remediation. ' lambda field technician notified pipeline manager, dan newell at 1158 (11:58am est). ' lambda energy field technician, dan keaton was the first person to arrive at junction station at approximately 1205 (12:05 pm est) he turned off the power to the pump. This stopped the leaking product. He then proceeded to isolate the suction and discharge valves of the pump and locked them out. This completely secured the leak and stopped the flow of product. ' matt allen notified the regulatory compliance manager, ben berthelot at 1222 (12:22am est) of the confirmed leak and its status. Leak has been isolated; response agencies were in route with approximately +/-350 barrels of crude oil inside an earth berm dike (believed to be compacted clay lined) at the pump and approximately +/-5 barrels of crude oil over the containment on the ground. ' onsite lambda personal notified the control room of the confirmed leak at junction station at approximately 1225 (12:25pm est). ' first vacuum truck from beckman services arrived onsite at 1225 (12:25pm est). They began sucking up free product from the ground and containment. ' ben berthelot contacted national response center [nrc report # 1415156] at 1254 (12:54 pm est). Supplemental report will be filed within 48 hours to update information once obtained. ' gfl arrived at junction station at approximately 1255 (12:55 pmest) an additional hydrovac was dispatched from kalkaska, and arrived at 1440 (2:40 pm est) ' ben berthelot contacted the michigan pollution emergency alerting system (peas) [incident # 31885] at 113 (1:13 pm est). ' notified (voice mail to cell phone) brian flickinger (michigan egle) at 115 (1:15 pm est). ' remediation efforts continued throughout the rest of the day. ' all crude oil was removed by use of vacuum trucks. A total of 85 barrels of crude oil was loaded on the vac trucks. The majority of this product was from the dike and approximately 2-3 barrels removed from the ground. This indicates we lost less than our original estimate of 350 barrels. We will update as more information becomes available. ' contaminated soil is being remediated and stockpiled on location for disposal. At the end of the day approximately 140 yards of material has been removed from the spill sight. ' lambda energy ehs director and otwell mawby (environmental consulting engineers) arrived onsite assess the spill and direct remediation efforts.
