While preparing the pipeline to send the ili tool down the line; operators contacted mvp control room to let them know that they were placing the valves in local operation. Mvp control room operated the main valve to the closed position, opened up bypass, closed kicker valve, control room confirmed valves 0% open and then on-site operations confirmed that the valves were closed then put in manual operations by operations. Drained barrel to sump . Bypass was open everything else was closed. When control room started the flow; liquid began spraying out of kicker valve. Operations immediately called control room to stop flow. It appears the cause was failure of the valve to fully seat. Update: elevation physically tested the valve and could not recreate the situation. The valve is fully operational. Programming of the valve closure was also tested and no issues identified. It appears that there was some paraffin debris that may have prevented the valve from fully seating. Elevation is updating the site specific procedure to address this potential issue, paraffin debris preventing full seating of the valve, in the future.
