On october 24, 2024, at 23:18 cdt, the edmonton control center received a seal fail alarm that was triggered while increasing the setpoints at flanagan terminal due to a line imbalance downstream. Line 59 was immediately shut down and a technician was dispatched to investigate the alarm. The technician arrived on site and at 23:57 cdt discovered crude oil leaking from the seal area of unit 4 onto the pump base and concrete floor. Approximately 15 gallons of crude oil was released and contained inside the pumphouse. The source of the release was identified as an inboard and outboard seal failure on unit 4. The unit was isolated, and cleanup of the release was completed by company personnel. Upon further investigation the cause of the release was due to an interlock tab breaking off which allowed the thrust nut to come unthreaded causing the shaft to work loose resulting in damage to both seals.
