Narrative
At approximately 12:10 on october 22, 2024, an fhr operator discovered a hydrocarbon odor and sheen on groundwater inside the check valve and block valve pit while performing a routine inspection. The fhr operator detected no lel readings inside the valve pits or at ground level. There was no indication of an active release at the time of discovery. The quantity of released product was estimated to be less than 1 gallon of diesel. At 15:00, a vacuum truck arrived on site and removed the water with sheen from valve pits. The valves and piping were inspected using a 360 camera and no leak was identified. On wednesday, october 23, 2024, fhr excavated the valves. No contaminated soil was discovered during excavation. On thursday, october 24, 2024, the coating on the check valve and block valve was removed. Once removed, fhr personnel noticed staining around the flange lid on the check valve. No staining was observed on the block valve. Onsite personnel retorqued the lid bolts on the check valve and discovered one bolt was loose. A function test was performed once all bolts were retorqued. There were no signs of an active release upon completion of the function test. The check valve was monitored while a repair plan was developed, and contractors were mobilized to the site to complete the repair. While some small staining was noted, no free product was present. On wednesday, november 6, 2024, a hot bolt injection repair was performed to seal the flange lid. No signs of staining have been observed since the check valve lid repair was complete.
