Narrative
On october 17, 2024, at 18:35 cdt, security personnel at griffith terminal notified operations of a strong petroleum odor at the front gate of the facility. Operations arrived onsite to investigate the odor and at 19:13 cdt discovered a release from the 202 manifold. The edmonton control center was immediately contacted, and lines 6a, 62, 64, and bp deliveries were shut down and the area was isolated. The release was estimated at 350 barrels of crude oil and was fully contained within the manifold's vicinity with no risk of migration off site. Regional and contractor resources were mobilized to begin response. Nrc criteria was determined to be met at 20:13 cdt, and nrc was notified at 20:56 cdt (nrc# 1414013). A 48-hour update to the nrc was made on october 19, 2024, at 20:40 cdt (nrc# 1414196). Upon excavation of the release area, the source of the release was a pinhole at the 6 o'clock position on 20" interconnect manifold piping. The section of pipe was cut out and the line was isolated. Approximately 270 barrels of crude oil was returned to the system, and contaminated soil has been removed from the release site. An internal investigation determined the release was caused by internal corrosion pitting on the interconnect piping. The results of the analysis indicate that the internal corrosion observed on the pipe occurred due to significant microbial activity. The failed piping was removed and blinded on both end connections as a permanent repair. Additional steps are being taken to prevent recurrence, including verifying the accuracy of the risk/threat evaluation and reviewing dead leg piping for similar threats.
