Narrative
During routine inspection the pump seal on the fast loop system was found to be leaking. Approximately 2 bbls (84 gallons) of diesel was released. About 42 gallons was released into concrete containment at the pump dike area with the remainder 42 being released to the ground. Local operator shut off the pump using the hoa switch and proceeded to close the inlet and outlet of the pump to isolate. It was determined the failure was due misalignment on the carrier bearing which caused the magnetic pump coupling to rub on the pump can causing a crack to occur in the can and allowing the release. The pump was replaced with a c- face motor to remove the carrier bearing. Temp and vibration monitoring was added to the magnetic coupling and additional pipe supports were added to the suction and discharge piping. Contaminated soil was removed and put in roll off boxes
