On 10/04/24 at approximately 18:00, a small flash fire occurred at lima mvpl terminal while welding to install a fabricated assembly in order to affect a pressure test the 20-inch ashland spur line. Tack welds were completed on the assembly and as contract crew was beginning the weld root pass a small flash occurred due to vapors passing mud plug. Line is non-piggable and dependent on drain up to evacuate line. Operations held a safety stand down after the flashfire. Cold cut tack welds to remove fabricated assembly for inspection and also, inspected the 20-inch spur line. After satisfactory inspection, proceeded with installation of fabricated assembly and pressure test. The investigation determined that the mud plug failed likely due to being installed too far in advance of the welding activities which may have allowed it to dry to an extent that the sealing capabilities were compromised and the plug was made with insufficient thickness for the diameter of pipe. The contractors qualifications were removed and involved individuals are being retrained.
