Narrative
On 10/4/2024 at 09:30 an enterprise technician was making a routine station check at shin oak energy east pump station when he observed an ice ball around a top vent on pump discharge piping. The pump station was immediately isolated and bypassed. Repair would require a section of piping to be cut out and replaced, resulting in repair costs exceeding $50,000. Results of metallurgical analysis on the removed section of piping indicated that the failure was likely due to a fatigue crack. To prevent recurrence, steps have been taken to assess and minimize effects of vibration. Nrc #1412781, 48-hour update #1412961.
