Narrative
On september 26, 2024, at approximately 7:45 am cdt, operations personnel performing routine duties in the cushing terminal noticed crude oil coming out of the drain piping on meter 31 at manifold 122. The cushing control center was notified, and the meter was isolated. The source of the release was a small corrosion area around the nipple on the meter that resulted in a release of approximately 21 gallons of crude oil. The released product was immediately cleaned up. The failed pipe was drained, and the nipple of the meter can was replaced. The investigation determined the cause of the failed nipple was due to a pinhole through the wall of the pipe at the 6 o'clock position. The nipple of the meter can was either poorly coated or only primed and not coated during installation causing external corrosion. To prevent reoccurrence, facilities integrity will visually review all nipples/drain valves of the meters in the cushing manifolds, assess the corrosion, and provide direction for remediation and or replacement.
