Narrative
At 11:06, the control room received a salem booster pump lockout first. This caused the clay city pipeline to shut down immediately upon the alarm coming in along with a list of other alarms regarding the shutdown. The control room also received a clay city to johnsonville delivery trip. The control room reset all the pumps and looked at the alarms in the historical. There were no active alarms holding out permissive to restart. The reasoning of the shutdown was not apparent and believed to be an issue in the programing since 2 automation techs (contract) were in the software fixing issues unrelated to this incident. The control room tried to restart the clay city to johnsonville pipeline. The control room received salem booster pump lockout, salem to clay city delivery trip, clay city delivery trip and a list of other alarms regarding the shutdown. The control room discussed with the automation contractor, but they were unable to tell US what was causing the delivery trips. At 11:50 the control room decided to attempt to start just the clay city line. The booster pump at clay city was started, but shutdown on delivery trip. The control room called the I&e supv. To get an I&e tech to go to clay city to look at a pressure transmitter. When he was informed we had a pressure transmitter reading zero, he told the control room they had taken all of those lines apart during a piping project and we may want to get a gauger over there to check it out first. At 11:58 the control room contacted the gauger to check the transmitter. At 12:15 the gauger informed the control room there was a leak on a pressure transmitter and that he was going to shut the valve to isolate the leak. The control room immediately called the qualified individual to initiate emergency response activities. Additional resources arrived on site at 12:45 to begin response activities. The tubing to the transmitter was repaired at 15:46. Operations restarted the clay city pumps at 4:20 under observation of the maintenance personnel. The response crews completed remediation activities of the site on october 11th. A third party materials lab completed a failure analysis on the tubing. The failure analysis concluded that "vibration in service from pressure variation was a likely cause of the cracking"
