Narrative
On september 17, 2024, at approximately 4:05 pm cdt, while technicians at the mokena station were performing unrelated work on line 14 assets, a plm laborer observed oil spraying out of the line 6 facility piping near the pressure transmitters. The edmonton control center was immediately notified of the release and line 6 was shut down. The affected equipment was isolated, and nearby field personnel responded to the release to begin cleanup. The source of the release was a 1-inch swagelok flex hose connecting mk-6-151-pit-1m/2m to the delivery pipe. The flex hose ruptured near the center of the hose braided length, releasing an estimated 40 gallons of crude oil. The failed flex hose was replaced with swagelok hardline tubing and all contaminated soil was removed from the release site. Further investigation determined that the integrity of the flex hose decreased over time due to the age of the asset leading to the eventual breakdown of material resulting in the release.
