Narrative
Targa personnel received call from targa's ngl control center about gas detectors indicating a possible leak at pump station 10. Supervisor dispatched personnel to site to investigate. Upon arrival, technician found product escaping from tubing fittings on poppet valve on pump 140. Tubing became detached allowing product to escape. Technician shut in pump station. Repairs were made and station returned to service within approximately 2 hours. Correction: revise part g sub-cause from improper installation to excessive vibration. Upon further investigation, it was determined pump vibration loosened the tubing and allowed the product release. Subsequent remediation included installation of clamps with rubber dampening to absorb the pump vibration and implementation of routine tubing inspection.
