Narrative
On 9/11/2024 at approximately 05:00, a contractor discovered a release of crude oil at the marshland manifold on inlet piping associated with #2 booster. The booster pump was shut down and the pressure relief valve was isolated which stopped the leak. The leak was determined to be coming from a crack in the 1.5-inch pressure relief line from the booster discharge line. Approximately 100 barrels of crude oil was released into containment. Cleanup crews removed free product and remediated contaminated soil. The 48hr nrc report was made as soon as possible after it was discovered that it had not been made w/in 48 hours of the original nrc report. An investigation was completed, and it was determined that a thermal pressure safety valve (psv) had setpoint level too close to the facility line normal operating pressure which allowed activation of the psv during routine startups of the associated booster pump. The psv failed to reseat which caused vibration during the continued flow through the psv and led to a crack in the schedule 80 pipe associated with the psv leading to the release. To prevent reoccurrence, the psv and associated piping has been replaced. The new psv has an increased setpoint to ensure it will not activate under normal operating conditions but will still protect the piping from thermal overpressure.
