On 8/27/24 @ 1450 hours, empire was notified there was a sheen on the water near psn-11724. Immediately, the producer pumping into this pipeline was instructed to shut-in & remain shut-in until further notice. The line was isolated & depressurized. Our smt was notified & nrc report no. 1409176 was filed followed by 1409245, 140936. An aerial flight was flown at 1700 hours w updated coordinates. Dsv joanne morrison arrived @ location on 8/29/24 @1315 hrs. Source: a ssti f/ a disconnected flowline decommissioned in 2018 by mcmoran oil & gas who failed to install & torque blind flange according to procedure along w/ leaving the isolation valve for the ssti assembly in the open position. Repair plan was developed w/tony wright's involvement. Blind flange was removed, cleaned & new r-50 o-ring & teflon coated studs & nuts were installed & the isolation valve was placed in the closed position. A pressure test was performed during daylight hours on 9/1/24 along with an aerial flight of area. Pipeline was returned to service on 9/1/24 at 1930 hrs.
