Narrative
On august 26, 2024, at approximately 1:43 pm cdt, a local technician performing daily rounds of the crane terminal discovered a release of crude oil coming from the tank mixer on tank 402. The edmonton control center was contacted and the tank was isolated. The tertiary containment was used to prevent further release into the secondary containment. The area supervisor arrived on site and determined that the release exceeded 5 gallons at 2:45 pm cdt. The source of the release was identified as a failed outer mixer seal causing the release of oil. The torque on the internal mixer seal was adjusted to prevent any further release. The railroad commission of texas was notified of the release at 3:26 pm cdt (report # ici-3153). Operations personnel were dispatched to begin cleanup of product and remediation. Approximately 20 gallons of crude oil was released. All contaminated soil has been removed and properly disposed of at an approved site. An investigation determined the mixer seal failed due to the shaft on the mixer having damage due to a bearing failure. The mixer is currently locked out and will be repaired at a future date. The nrc was notified on september 25, 2024, at 1:42 pm cdt (report # 1411888) as costs are expected to exceed the $50,000 nrc reporting threshold. A 48-hour update to the nrc was made on september 27, 2024, at 12:31 pm cdt (report # 1412079).
