Narrative
On the morning of 8/27/2024 pipeline operations were in the process of liquid filling the daytona 24" ngl pipeline for initial commissioning. At approximately 8:20 am, at mainline valve (mlv) 28, scada indicated an expected pressure increase to approximately 725 psig, then immediately dropped off. A nearby landowner heard the release and notified targa operations at approximately 8:30 am. Operations arrived on location at approximately 8:45 am and isolated the upstream valve, shutting off the source to the stainless-steel tubing leak. The 3/4" upstream valve that was isolated is located on the bypass line for the mainline valve and provides the source to the pressure transmitter. Upon investigation, it was determined that the stainless-steel connection located at the transmitter was not tightened enough to adequately secure the tubing to the ferrule. As a result, the tubing was forced out of the connection at the point pressure reached approximately 725 psig. New stainless-steel tubing and connections were installed, and transmitter was placed back into service the same day.
