Narrative
A contract operator was instructed to check the pig sig on the receiver to see if a pig had landed. Instead of just checking to see if the pig sig had tripped, the operator also opened the barrel. The operator had not drained the barrel, so when it was opened the contents of the barrel spilled into the secondary containment and onto the ground. The contractor onboarding process is being improved to more strongly emphasize the prohibition of any operator from performing work or activities that they are not specifically authorized and trained to perform.
