Narrative
On 8/11/24 at 23:30, a leak was discovered in the mainline pump strainer area of the el reno to cushing system #2 line at el reno station. The area was isolated and the line shutdown. The investigation determined the leak was coming from a threaded 1/2-inch pvit fitting. Approximately 5 barrels of crude oil was released to the ground. Released product was cleaned up and impacted soil remediated. Failed fitting was replaced and line restarted. The cause of the leak is still under investigation. A supplemental report will be submitted once the investigation is complete. Supplemental report filed on november 15, 2024: during the accident investigation, it was determined that sudden heavy rainfall cause increased settling of the soil in the area of the fitting. Prior to the leak, the ground had previously been excavated and backfilled. Weather conditions were in drought prior to the rainfall. With the age of the fitting and the sudden rainfall causing settling, the fitting leaked. The threaded fitting was replaced with a single stainless steel line to prevent future reoccurrence. The single stainless steel line will prevent multiple connection points that could serve as potential leak points.
