On august 12, 2024, a line locating contractor working near the cyclone ctf to thor 10-inch above ground t piping noticed a leak coming from a valve. At approximately 12:38 p.M., western midstream operations pipeline foreman dispatched field personnel to assess the area, field personnel arrived on site and stopped release. On august 12, 2024, at 2:51 p.M. Western midstream confirms crude oil release amount of .23 barrels released. Western midstream determined the lever on ball valve was found in a partial opened position resulting in the primary cause of release and secondly the valve plug was loose both contributed to release. The true cause of lever on ball valve opening is unknown, however in conclusion, we believe this valve was likely opened by livestock as valve is in a rural area. Western midstream in error reports event twice to the nrc (nrc report numbers: 1407937/ 1407940)
