Narrative
On august 7, 2024 at 13:57 cst, during routine pipeline repairs on the west red line near mile post 36.5, a burn-thru occurred resulting in a leak. After investigation, it was discovered the welder did not properly follow company standards and the inspector failed to provide oversight to ensure protocol was followed. Oq's were suspended immediately for the welder and inspector performing the work. The damaged pipe was cut out and replaced. Pipe replacement costs exceeded $50,000 which required an nrc notification.
