Narrative
On 08/08/24 at approximately 07:15, during the morning walkthrough, a local operator employee discovered a leak on the 16-inch strainer bypass piping at mayersville pump station. The control center was immediately notified and the station was isolated although at the time of discovery the pipeline was down on normal schedule. The leak area was further isolated and an investigation determined the leak was coming from a pinhole leak at the 6 oclock position on the strainer bypass piping. The leak point was repaired using a 16-inch mechanical clamp. Approximately 15 gallons (0.4 bbls.) of crude oil was released. Cleanup contractors removed product and remediated impacted soil. The failed section has been cut out and replaced and sent to a laboratory for examination. Supplemental & final: the lab analysis determined the cause of the accident was due to internal corrosion. To prevent reoccurrence, continuous treatments upstream of this station have been implemented and the volume of product to flush the line was increased.
