Narrative
On august 5th, 2024, at approximately 10:55 cdst, enterprise employees removed tank 1022 vertical booster pump for maintenance. Employees then observed product re-entering the evacuated can from a one-inch threaded plug on bottom of the pump. Upon investigation it appeared that the original sealant applied to the plug during install had degraded. For repair the failed plug was replaced with a solid steel core plug with approved thread sealant. Approximately 4 gallons were released and have been cleaned up.
